The card issuance process is automatic.
You will need to enter your request in the online form, of which you will find here.
The details you have written on the form will be identified with the information that you have given to us, and if you are an active customer; either directly insured, a dependent member, or an employee that is insured as a part of a group policy, the membership card will be automatically sent to your email address.
In the event that we are not able to identify your details automatically, we will do this process manually and we may have to contact you in the case that some of your details might have changed.
The membership card is strictly personal.
